Patient Forms
At West Michigan Dermatology & The Skin Revitalizing Center, we believe superior customer care begins before you arrive at the doctor’s office. That’s why we provide all the necessary forms for our patients. If you prefer to complete our forms in the convenience of your own home or office, just print the appropriate form(s), fill them out, and bring them with you to your first appointment.
Payment Options
We accept payment by cash, check, or credit card. Flexible Spending Account (FSA) cards may be used for medical payments, but are not accepted for cosmetic procedures or product purchases.
CareCredit is easy to apply for with a simple application and quick online approval. The financing allows you to begin your treatment immediately and then pay for it over time with low monthly payments. Various payment plans are available to suit your needs.
